HOW OUR OFFICE COLLECTS, USES AND DISCLOSES PATIENTS’ PERSONAL INFORMATION

- to offer and provide treatment, care and services in relationship to the chiropody/podiatry scope of practice
- to communicate with other health-care providers, including specialists who have referred you or to who we refer you and your family doctor

- to allow us to efficiently follow-up for treatment, care and billing
- for teaching and demonstrating purposes on an anonymous basis
- to prevent insurance fraud by confirming treatment dates, costs and services rendered with insurance companies upon request from them
- to comply with legal and regulatory requirements, including the delivery of patients’ charts and records to the College of Chiropodists of Ontario in a timely fashion, when required, according to the provisions of the Regulated Health Professions Act
- to permit potential purchasers, practice brokers or advisors to evaluate and / or conduct and audit of the foot practice
- to deliver your charts and records to the chiropodist’s insurance carrier to enable the insurance company to assess liability and quantify damages, if any
- to prepare materials for the Health Professions and Appeal Review Board (HPARB)
- to invoice for goods and services and process credit card payments
- to collect unpaid accounts
- to assist this office to comply with all regulatory requirements and to comply with the general law
YOUR DATA
In order to facilitate your care, The Footcare Centre uses an office-based practice management system. Data backups are stored on remote cloud servers to facilitate continuous treatment in the event of a system failure at our office. Due to the nature of internet cloud computing, information may cross borders. We will always ensure that your data is stored to the highest level of protection under Canadian law. Please be advised that we never store any credit / debit card details after your transaction.
By accepting treatment at this office or using this website, you have agreed that you have given your informed consent to the collection, use and/or disclosure of your personal information for the purposes that are listed.

Online Bookings
All bookings made via the online bookings function on this website are provisional until confirmed by our reception team. We will confirm acceptance of your appointment by email or telephone call. Our service level will be to contact you within 1 working day. If you have not heard from us within this time, then please call us on 905-357-0214 to check that we have received your online booking. Appointments can only be booked online and not cancelled. Please see the section below on our cancellation policy should you need to cancel your appointment.
Cancellation Policy
We appreciate that sometimes you need to make changes or cancel pre-booked appointments with us. There are several ways you can do this. You need JavaScript enabled to view it.. We would kindly request that you give us a minimum of 24 hours notice of a change or cancellation where possible. This will allow other patients waiting for an appointment to be seen. If you are unable to give us 24 hours notice of a change or cancellation, then a charge may apply.
